ECG Hallucinations: The Rhythms You See and the Rhythms You Miss
ECG Weekly Workout with Dr. Amal Mattu
HPI
A patient presents with a regular narrow-complex tachycardia. The computerized interpretation identifies sinus tachycardia, and the rhythm appears superficially compatible with that diagnosis when lead II is reviewed. However, closer inspection of the remaining leads reveals a different atrial rhythm.
Before watching this week’s workout, review the ECG carefully and consider:
-
- Is this truly sinus tachycardia, or are atrial deflections being overlooked?
- Which lead is most likely to reveal atrial flutter when lead II is nondiagnostic?
- What finding suggests that an apparent T wave may actually contain a buried flutter wave?
