Example of a regular, REALLY wide complex tachyarrhythmia (QRS ~ 200 ms or a full box wide) mimicking ventricular tachycardia. Note the presence of P-QRS complexes seen best in leads V1 & V2. This is NOT VT, rather sinus tachycardia with severe hyperkalemia. The patient was stable and treated with lidocaine for suspected VT. The patient decompensated, had a bradyasystolic arrest, and could not be resuscitated.
