A 45-year-old woman presents to the ED with severe chest pain. She is minimally cooperative when trying to obtain a history and is requesting pain medication for severe pain. Physical exam is non-diagnostic. The following ECG is obtained on arrival with no prior ECGs available for comparison:
Before watching the video, look closely at this ECG and ask yourself:
- What ECG abnormalities do you notice?
- What is the isoelectric baseline of the ECG upon which you evaluate ST segments?
- Is this ECG more consistent with STEMI or acute pericarditis to you?
