UMEM Potpourri ECG Cases with Dr. Amal Mattu


HPI

A 69-year-old man arrives at the emergency department with shortness of breath and profuse sweating. The ECG obtained below shows sinus tachycardia at a rate of ~ 130 bpm with a right bundle branch block (RBBB). The initial interpretation, both by the ECG machine and the clinical team, labels it simply as sinus tachycardia with RBBB. The patient soon after develops cardiogenic shock.

Before watching this week’s workout, closely examine the ECG and ask yourself:

    1. In the presence of RBBB, what does ST elevation in leads V1 and V2 signify?
    2. Why might sinus tachycardia be a red flag in a patient with suspected STEMI?
