A 66-year-old man is transferred to the ED from an urgent care where he presented for shortness of breath and was diagnosed with heart failure. He was recently discharged s/p PCI for anterior MI 2 days ago. He is placed on the monitor, and the following rhythm strips were obtained:

Before watching the video, look closely at this ECG and ask yourself these questions:

  1. What is the rhythm?
  2. What is the most likely cause?
  3. How would you manage this rhythm abnormality?
